Ketamine-Assisted Psychotherapy (KAP) is an integrative approach that combines the use of ketamine with psychotherapy, aiming to facilitate more frequent breakthroughs and enduring symptom relief for suitable patients. In this therapeutic partnership, our therapists handle the psychotherapeutic aspects of your journey, while the medical team from Journey Clinical takes care of all medical-related concerns.
This includes assessing your eligibility for the treatment, designing a personalized treatment plan tailored to your needs, prescribing the appropriate dosage of the medication, and closely monitoring your progress and outcomes.
The following details about KAP are provided to help you better understand this treatment modality and to assist you in deciding whether it could be a beneficial option for you.
Ketamine Explained: Ketamine is a legal, safe and potent medication used to manage a range of mental health disorders such as depression, anxiety, and PTSD. It has swift antidepressant and mood-elevating properties, with effects often noticeable within 1-2 hours post-treatment and lasting up to a fortnight. Ketamine operates by inhibiting the brain’s NMDA receptors and stimulating AMPA receptors, which are believed to establish new synaptic connections and strengthen neural circuits that control stress and mood. Ketamine has also been found to increase overall neuroplasticity for sustained symptom relief.
The administration of Ketamine can be done through various methods, including IV infusion, intramuscular injection, nasal spray, and sublingual lozenges. In my collaboration with Journey Clinical, we exclusively use the sublingual lozenge form.
Ketamine Experience: The effects of ketamine, typically perceived as pleasant by most patients, last around 45 minutes. These effects can induce a sensation of being “detached” from your body and facilitate perceptual shifts that often feel expansive. Your motor and verbal skills will be diminished, hence you’ll be lying down in a relaxed position during the experience. Once these effects subside, the remainder of our appointment will be devoted to providing you space to process and discuss your experience. While articulating what transpired during the experience may seem challenging, patients often find the insights gained to be crystal clear. Research shows that the benefits to mood and neurological growth can persist for up to two weeks after a Ketamine session.
How Does Ketamine-Assisted Psychotherapy Work?
- Initial Consultation with Journey Clinical: You’ll schedule an initial assessment with a clinician from Journey Clinical’s medical team via zoom after a referral from our KAP therapist is made. They will review your medical and psychiatric history, educate you about the treatment, and assess your eligibility for KAP. If the medical team at Journey Clinical deems you suitable for KAP, they will formulate a personalized Ketamine prescription and outcome monitoring plan. The medical staff of Journey Clinical will issue a ketamine prescription, and a small quantity of oral ketamine will be dispatched to your residence, sufficient for the first two KAP sessions. You will be tutored on how to check your vitals and self-administer the ketamine lozenges by Journey Clinical’s medical team prior to our KAP sessions.
- Preparation Sessions: Once you receive your ketamine lozenges, you and your clinician will allot time for the KAP preparation, dosing, and integration sessions. Preparation sessions will be arranged like regular therapy sessions before the KAP dosing session. The objective of these sessions is to familiarize you with the process and set goals for the joint KAP sessions.
- KAP Dosing Session: A typical ketamine dosing session lasts between 1-2 hours and can occur either in person at our office (highly recommended) or remotely via telehealth. During a dosing session, you will self-administer your ketamine lozenge either at our office or your home. You’ll be reclining comfortably with an eye mask on, listening to soothing music. Though a KAP dosing session might be mostly an inward experience, our KAP therapist will be there throughout to provide a safe space and support as required.
- Integration Sessions: After KAP dosing session, we will conduct multiple integration therapy sessions to explore the memories, thoughts, and insights that surfaced during your dosing session, and to prepare for the subsequent dosing session.
- Follow-up Consultations with Journey Clinical: Post the initial KAP session, the medical team at Journey Clinical schedules regular follow-ups with you to monitor outcomes and prescribe ketamine lozenge refills, as required. The frequency of these follow-ups depends on your individual treatment plan, but at a minimum, they occur once per quarter.
Follow-up Consultations with Journey Clinical :
After the first Ketamine-Assisted Psychotherapy (KAP) session, the medical team at Journey Clinical will establish a routine of regular follow-ups with you. These check-ups serve to monitor your progress, evaluate the effectiveness of the treatment, and prescribe refills of the ketamine lozenges when needed.
The frequency of these follow-up consultations is tailored to match your individual treatment plan. However, they are scheduled to occur at least once every quarter as a standard practice. This consistent monitoring ensures that the treatment remains adaptive to your evolving needs and continues to facilitate your journey toward improved mental health.
